The San Diego Pharmacist Resource and Research Network is conducting a study to discover the best pharmacists/pharmacy practices in San Diego. Network members were asked to nominate pharmacists for their innovative practice. These pharmacists were interviewed by UCSD students. A short video footage of each pharmacist interview is now posted via the network’s virtual community hosted on the SciVee website. Sharp HealthCare is the largest Integrated Delivery Network (IDN) in San Diego. The purpose for the initiation of the Outpatient Orthopedic Coumadin Clinic was to maintain the consistently excellent clinical outcomes achieved by the current model where pharmacists provide medication therapy managment for patients taking warfarin. The physician medical groups at Sharp Memorial Hospital (SMH) were interested in starting the Pharmacy Orthopedic Surgery Anticoagulation Service, especially since the pharmacy departments had been successfully managing warfarin therapy for hospital patients for several years. This is an excellent example of where the physicians and pharmacists successfully collaborate to improve patient care in an integrated health setting.
